PATIENTS AND VISITORS GUIDE

 A tertiary level training general hospital with a capacity of 180 beds 


Admission is so easy in just 3 steps....


Step 1

Patient proceeds to the Triage Area for evaluation and registration. For emergency cases, patient proceeds directly to treatment. Patients with “Direct-To-Room Order” should make prior arrangement with the Admitting section.


Step 2

Resident Doctor assesses patient and refers to the Attending Physician.  If for admission, the attending Physician orders for patient admission through the Resident Doctor. When admission is not necessary, the Resident Doctor prescribes appropriate management and Nurse gives discharge instructions before a patient goes home.


Step 3

Patient for Admission:  Folk completes the admission process and accompanies patient for transfer to the assigned room. Patient is now officially admitted.

UPON ADMISSION

Where can a PhilHealth Member get the form to be accomplished? 

  • The patient will be given the CF1 form to be accomplished by the PhilHealth Member and Employer. A list of required documents that need to be submitted upon discharge will also be given. 


DURING YOUR STAY

Have the documents checked by PhilHealth staff ahead of time to avoid delay and non-deduction of benefits (PhilHealth section is located at the Business Office – Window 1).


What are the important requirements for PhilHealth Benefits?

  • CF1 – Claim Form 1: Accomplished and originally signed by member and employer
  • Proof of payment of premium contributions at least 6 months paid prior to confinement


What is the additional requirement for a Government Member?

  • Claim Form 1 duly signed by the authorized person in the office/company


What are the additional requirements for a Private Member?

  • Claim Form 1 duly signed by the authorized person in the office/company
  • Proof of Payment of Premium Contribution (itemized)


What is the additional requirements for a Indigent/Sponsored Member?

  • Original Certificate of Eligibility with validity period


What is the additional requirement for a Self-Employed/Individually Paying Member?

  •  Proof of payment of premium contributions 6 months paid prior to confinement with official receipt


What is the additional requirements for an Overseas Worker?

  •  Copy Of MDR With Payment Information


What is the additional requirement for a Senior Citizen/Lifetime Member/Non-Paying Pensioner? 

  •  No requirements needed as long as previous category is updated


UPON DISCHARGE

What should the Member do upon Discharge? 

  • Submit all the complete required PhilHealth documents approved by PhiHealth staff upon discharge at the billing section.

We encourage you to speak openly with your   health care provider, take part in your treatment choices, and protect your   own safety by being well informed and involved in your care. As a patient at St.   Paul’s Hospital of Iloilo, Inc., you have the following rights:                                                                                                          

Hinihikayat namin kayong maging bukas sa pakikipag-usap sa inyong health care provider, makibahagi sa inyong mapipiling paggamot, at maprotektahan ang inyong kaligtasan sa pamamagitan ng pagiging maalam at na kikisangkot sa inyong kalusugan. 


1. You have the right to receive considerate,   respectful and compassionate health care in a safe setting regardless of your   age, sex, gender, religion, ethnicity, political affiliation, disability or   capacity to pay free from all forms of abuse, neglect, or ill treatment.

 (May karapatan kong tumanggap ng   mapagsaalang-alang, magalang at matulunging health care sa ligtas na Iugar   anuman ang iyong edad, sex, kasarian, relihiyon, etnisidad, kasapiang   politikal, kapansanan, o kakayahang makapagbayad, na malaya sa lahat ng anyo   ng pang-aabuso, kapabayaan, o maling pagggagamot.)


2. You have the right to be assigned to a   competent doctor/resident physician and be told of the names of all health   care team members who are qualified to provide diagnosis, treatment and   medical advice. Likewise, you have the right to know your hospital and   physician fees, and receive information about the possibility of financial   assistance.

(May karapatan kong mabigyan ng mahusay   na doctor at maipaalam ang mga pangalan ng lahat ng kasapi ng health care   team na kwalipikadong susuri, gagamot, at magbibigay ng payong medikal.   Gayundin, mayroon kong karapatang malaman ang iyong bayarin sa ospital at   doktor, at makatanggap ng impormasyon hinggil sa posibilidad na tulong   pinansyal.)


3. You have the right to notify a family member   or person of your choice and your chosen doctor of your admission to the   hospital.

(May karapatan kong ipaalam sa miyembro   ng iyong pamilya o sa sinuman at sa iyong napiling doktor ang iyong admisyon   sa ospital.)

 

4. You have the right to have   someone remain with you during your hospital stay unless it compromises your   or others’ rights, safety or health.

(May karapatan kong magkaroon ng kasama   sa iyong pananatili habang nasa ospital maliban kung nakokompromiso nito ang   karapatan, kaligtasan, o kalusugan mo o ng iba.)

 

5. You have the right to exercise   your spiritual and cultural beliefs within the capacity and rules of the   hospital/medical center.

(May karapatan kong isagawa ang iyong espiritwal   at kultural na paniniwala na umaayon sa kakayahan at mga tuntunin ng   ospital.)


6. You have the right to be informed and give consent before   any non-emergency procedure or research/experiment or to refuse such.

May karapatan kong malaman at   makapagbigay ng pahintulot bago ang anumang prosidyur o pamamaraan na   di-emergency o pagsasaliksik/ eksperimento o tanggihan ang naturan.

 

7. You have the right to privacy and   confidentiality of your medical records according to laws, as well as in care   discussions, examinations, and treatments and the right to see or get a copy   of your medical records except those records restricted by law.

(May karapatan ka sa pagiging pribado at   kumpidensyal ng iyong ulat medikal alinsunod sa batas gayundin sa mga   diskusyon sa pangangalaga, eksaminasyon at paggagamot, at ang karapatang   makita o mabigyan ng kopya ng ulat medikal maliban kung ang natukoy na ulat   ay di-pinahihintulutan ng batas.)

 

8. You may request for an escort   during physical examinations.

(Maari kong   humiling ng kasama habang isinasagawa ang pisikal na eksaminasyon.)

 

9. You have the right to be   represented by someone (assignee) to decide on your behalf when the   circumstances warrant.

(May karapatan kong katawanin ng iba   upang magpasiya para sa iyo kung hihingiin ngpagkakataon.)

 

10. You have the right to ask about   and be informed of the complaint process and express grievances without fear   of recrimination or reprisal. You are encouraged to speak directly to the   health care provider involved in your care.

(May karapatan kong magtanong at mala   man ang proseso ng pagrereklamo at ipahayag ang mga hinaing nang walang takot   sa igaganting-paratang o paghihiganti. lkaw ay hinihikayat na direktang   makipag-usap sa health care provider na nangangalaga sa iyo.)


If there are issues   not resolved to your satisfaction, or if you would like the help of someone   not immediately involved, talk to the Client Relations Staff or to the Chief   of Hospital or Hospital Administrator. If you find the above avenues   unsatisfactory, you may choose to file a formal grievance with following   agencies:

Kung may mga isyung hindi naaayon sa iyong inaasahan, o   kung nais mo ng tulong mula sa isang taong hindi direktang sangkot,   makipag-ugnayan sa Kawani ng Ugnayang Pampasyente, sa Direktor ng Ospital, o   sa Tagapangasiwa ng Ospital. Kung ang mga nabanggit na paraan ay hindi pa rin   kasiya-siya para sa iyo, maaari kang magsampa ng pormal na hinaing sa   sumusunod na ahensya:

 St. Paul’s Hospital of Iloilo offers paid parking spaces conveniently located at the main facade and at the back parking building with helipad, can accommodate more than 265 vehicles.
